FAA changes San Francisco landing procedures after A320 near miss
https://www.flightglobal.com/news/artic ... -a-440380/Controllers at the airport will no longer permit pilots to perform visual approaches at night "when an adjacent parallel runway is closed", the agency tells FlightGlobal.
"When these conditions prevail, our controllers issue pilots instrument landing system approaches or satellite-based approaches, which help pilots line up for the correct runway," says the FAA in a statement.
The agency adds that San Francisco's tower management now requires two controllers be working in the tower "until the late-night arrival rush is over".
Pilots of the Air Canada A320 told US investigators they thought the lighted runway was 28L and that taxiway C was runway 28R, the NTSB said

Much too close. Again, they caught this in time, but that's a matter of timing / luck more than anything. Over the course of the few years that we've been discussing such incidents on this board, such approaches are alarmingly common, including one by Han Solo, who's supposed to be an experienced model aviation spokesperson.
While I don't think using nav / GPS / computer aids is a solution either, but at least they recognized that if one runway is out they should do something about it , and at least another controller during those times is a start. Because the all-too-conditioned human brain will simply shift reference points.
I'm thinking they probably need to put a big red "X" over the runway that is out and another big blue "X" over the taxiway, just so to make it very clear.

IRC, the big red X in closed runways is already a Marks regulation.

Okay, but tell me, it was night, the approaching plane saw the waiting planes on the runway only when it was already pretty low, maybe even below decision height (looks a little foggy to me). Soooooo, just wondering why has the ATC a plane waiting there while he had one on short? I mean, okay, California legalized Cannabis, but I think, smoking MJ in the tower should be forbidden, seriously!
